List of acceptable ID and Address proofs of claimant

Insurance Contract with individuals 1. Passport;
2. PAN Card;
3. Voters ID Card;
4. Driving License
5. Proof of possession of Aadhaar number issued by the UIDAI.
6. Job Card issued by NREGA duly signed by an officer of the State Government
7. Aadhaar Card/letter issued by UIDAI

Provided that where simplified measures are applied for verifying the identity of the clients the following documents shall be deemed to be ‘officially valid documents’

8. Photo ID Card issued by Central/State Govt, Statutory/Regulatory Authorities, Public Sector Undertakings, Scheduled Commercial Banks and Public Financial Institutions;
9. Letter with Photo attested by Gazetted Officer;

Provided further that where simplified measures are applied for verifying the limited purpose of proof of address of the clients, where a prospective customer is unable to produce any proof of address, the following documents shall be deemed to be ‘officially valid documents’ :

10. Utility Bills not older than 2 months of any service provider(electricity, telephone, post-paid mobile phone, piped gas, water bill)
11. Property/Municipal Tax Receipt
12. Bank account or Post Office savings account statement 13. Pension/Family Pension Payment Orders of retired Govt/PSU employees
14. Letter of Allotment of Accommodation issued by Central/State Govt, Statutory/Regulatory Authorities, Public Sector Undertakings, Scheduled Commercial Banks, Public Financial Institutions and listed companies. Similarly, leave and license agreements with such employers allotting official accommodation.

Provided also that in case the officially valid document presented by a foreign national does not contain the details of address, in such case the documents issued by Government departments of foreign jurisdiction and letter issued by Foreign Embassy or Mission in India.

Provided that the client shall submit updated officially valid document with current address within a period of three months of submitting the above documents.
